 My bespoke, one-on-one coaching program can offload some of your pressure, while providing a useful resource for your patients. My job is to coach people through the lifestyle changes needed to better manage their health; lose weight, implement an exercise regimen, improve their sleep, manage stress, balance their hormones, and learn healthy sustainable habits. I can help your patients on their journey and ensure they understand the care plan you created for them. I work closely with them to implement the complex lifestyle changes that are often required to make significant improvements in their health.

  • Accountability and attention are provided in our regularly scheduled 45 minute (in-person, telephonic or virtual) sessions to make healthy lifestyle changes and overcome any barriers to change.

  • They will be empowered with guidance, strategies and tools to improve their our health and wellness.

  • A step-by-step action plan will be designed to fit with their lifestyle to meet health goals.

  • Guidance through the healthcare plan you have recommended so they can return to you with improved markers; lowered lipid levels, improve blood pressure readings, weight loss, balanced hormones, etc.

How you can refer your patients to me:

  • With patient permission, email your referral to livewell@rhiana.com including their name, email and phone so that I can reach out to discuss their options. A brief reason for referral also assists in my understanding of patient situation and needs.

  • Send your patient here at rhiana.com to set up a discovery call.

  • Fill out the patient referral form and I will reply within 1-2 business days to begin the intake process

Rhiana’s positive-based and empathetic approach to wellness comes from years of experience, and her own struggles to overcome her unhealthy relationship with stress and alcohol. She is a graduate of the University of Connecticut and the Dr. Sears Wellness Institute (now ACE), a Certified Yoga Teacher, and a National Board-Certified Health & Wellness Coach. (NBHWC).
